Steve Tague Studios, LLC d/b/a Rowed to Fitness (hereinafter the “Personal Trainer”) is registered as an ABN with the State of Oregon. This waiver (hereinafter the “Waiver”) constitutes the full and complete understanding and agreement of the parties hereto.

I am aware that there are significant risks involved in all aspects of physical training and/or active metabolic testing (VO2 Max Test). I wish to participate in the fitness program as set forth by the Personal Trainer and willingly assume full responsibility for the risks that I am exposing myself to and accept full responsibility for any injury or death that may result from participation in any activity or class while at, or under direction of the Trainer. I understand that even when reasonable precautions are taken, accidents may happen. Therefore, in exchange for the Personal Trainer allowing me to participate in certain activities, I understand and expressly acknowledge that I release the Personal Trainer, Rowed to Fitness, and Steve Tague personally from all liability for any injury, loss or damage connected in any way whatsoever to the the Trainer's activities whether on or off the premises of the Trainer. I understand that this release includes any claims based on negligent action or inaction of the Trainer’s employees, contractors, members, clients or guests. I have read and I am voluntarily signing this authorization and release.
